Patient Rights & Responsibilities

AS A HOME MEDICAL EQUIPMENT CLIENT YOU HAVE THE RIGHT TO:

  • Be given appropriate and professional quality home medical equipment services without discrimination against your race, creed, color, culture, sex, national origin, sexual preference, lifestyle choices, psychosocial and spiritual values, philosophical personal values/preferences, religious beliefs, handicap, or age. We support the right of each patient to personal dignity.
  • Be treated with respect and courtesy by all staff that provided home medical equipment to you and be free from mental, physical, sexual and verbal abuse, neglect and exploitation.
  • Choose your provider
  • Make informed decisions regarding your case.
  • Whenever possible, communication with home care staff is in a language or form you can reasonably be expected to understand.
  • Refuse treatment
  • Receive information about all financial responsibilities.
  • Receive complete and competent service by a trained individual.
  • Receive equipment/service in a timely manner.
  • Consent to and be given information prior to any experimental treatments.
  • Participate in the development and revision of you individualized plan of care or service.
  • Voice concerns or complaints without fear of reprisal.
  • Have your privacy respected. All information about you and your needs are held confidential.
  • Have you advance directives honored.
  • Have your right to security respected
  • Have your property respected.

 

AS A HOME MEDICAL EQUIPMENT CLIENT YOU HAVE THE RESPONSIBILITY TO:

  • Give accurate and complete health information concerning your past and present illnesses, hospitalizations, allergies, medication and other pertinent facts.
  • Assist in developing and maintaining a safe home environment.
  • Inform PharmCo when you will be unable to keep a home medical equipment delivery appointment.
  • Participate in the development and update of you care plan.
  • Adhere to your home medical equipment plan of service/treatment.
  • Request further information concerning anything you do not understand.
  • Inform PharmCo if you are unable to meet your financial obligation for payment of equipment.
  • Contact your physician whenever you notice a change in your condition
  • Inform PharmCo of concerns or problems regarding your equipment or any staff member.
  • Inform PharmCo if admitted to the hospital.
  • Inform PharmCo of any changes in residence, phone number, or physician.
  • Inform PharmCo if you acquire a communicable disease.
  • Inform PharmCo if you have ever rented or purchase equipment from any other home medical equipment company prior to date of service.
